We are a multicultural Medicaid Support Coordination agency that provides Case Management services. We support adults with intellectual, mental health and developmental disabilities who have been diagnosed with a disability before the age of 21. We serve all New Jersey Counties and pride ourselves on helping those that qualify obtain supports to accomplish their goals. The Medicaid Fee-For-Service comprehensive waiver offers over 20 different types of services. The First Lynx Team honors of our ability to discover, review, apply, and visit resource providers of your choice and go above and beyond to also connect you to community resources.

We have two locations within New Jersey. Our administrative office is in Hoboken, New Jersey and our visit/meeting office is in Newark, New Jersey. If you choose us as your support coordination provider, our committed team will go the extra mile to create the perfect individualized support plan (ISP) for you. We will design, approve, coordinate, and monitor the best plan. This way, you will be able to discover your abilities and the options to achieve your lifetime goals. 

If you currently attend High School, we also assist families and their loved ones. Individuals with disabilities may attend High School until the age of 21 and the transition period to start reviewing life after school normally begins between the age of 18-21.
