DDD ensures that individuals with intellectual and developmental disabilities will be given the opportunity to receive quality support and services.

Who Is Division of Developmental Disabilities (DDD)

First Lynx will introduce to you DDD. The New Jersey Department of Human Services’ (DHS) Division of Developmental Disabilities (DDD) ensures that adults with developmental/intellectual disabilities will be given the opportunity to receive quality support and services.


This way, they can participate meaningfully within their communities, make their own decisions, and live as independently as possible. Interested individuals can access these services through one of these two Medicaid waiver programs: the Supports Program (SP) or the Community Care Program (CCP).


These programs provide public funding for certain services to assist all eligible adults in New Jersey. The option to participate within the Supports Program (SP) or the Community Care Program (CCP) is completely voluntary. To qualify for DDD services, an individual must:

1. Be a New Jersey Resident

2. Be Medicaid eligible

3. Prove that he/she has a chronic intellectual and/or physical impairment that

    • Manifested before age 21;
    • Is lifelong; and
    • Substantially limits him/her in at least three of these life activities:
4. Self-Care
5. Learning
6. Mobility
7. Communication
8. Self-Direction
9. Economic Self-Sufficiency
10. Ability To Live Independently
